     2 Maids of Honor and one best man?  

I cannot choose between my sister and best friend. My fiance really doesn't want 2 best men. Is it fair to have 2 maids of honor and only one best man?

It is fine to have 2 maids of honor and one best man. Note that maid of honor is the title given to women who are single, never married. So, if either of these women are married, divorced or widowed, their title would be matron of honor.
