How to list ill grandmother on the wedding program

My grandmother has become very ill in the last couple of months and may not be strong enough to attend my wedding. How can I acknowledge her at the ceremony and in the wedding program?

HELP

Dear Amazon,

It may seem as if she has passed if this isn't done very carefully. You could light a candle for her during the ceremony and have this listed in your program as a lighting of your grandmother who is ill and could not be with you today. Your officiate could also mention why you are lighting the candle.

Best wishes,
