Parent's names included on church program for wedding

I need to know proper wedding ceremony program wording. The parents of the bride will be attending the wedding and their names will be on the program.

Father of the groom has chosen not to attend. Should his name be on the program as father of the groom?

Mother of the groom is deceased. Should her name be on the program as mother of the groom?
