We are having an open bar at the reception and we would like to include a slip indicating that we would like our guests to make a donation to a chosen charity. We don't want to make it sound that they have to pay for their drinks, but that if they made a contribution we would be grateful

This would be inappropriate on many levels. We shouldn't impose a charity on our guests, especially during a reception. And, we never ask for money. Plus, asking for the donation for a drink is like asking your guests to pay for their drinks. We don't do this as hosts.
It seems that you want to promote a charity, which is a great idea. We just don't do this during a reception.
