Listing Bride's name in a Second Wedding Announcement

This is my second marriage and I am not quite sure how to word my wedding announcement for the local newspapers in regards to how my name should appear. I currently use my 1st husbands last name due to the fact that we have a child together. That of course will change after I get married in October. On the forms for the newspaper they ask for the brides full name. Should I fill it out the following way:

First name, maiden name, current last name?

I thought that by including the maiden name that maybe more people would recognize who I am.

What should I do?

Listing your name as you have indicated seems reasonable.
