Wedding date and Reception has changed

I have ordered and received my wedding invitations. Because of legal matters, I had to change the wedding date and the reception place. Is it proper to add an insert with the new changes? If so, how would it be worded?

Yes, this would be best. The wording should be simple and clear. It doesn't have to follow a certain format.
