     Wording wedding announcement to include maiden vs married name  

I will send annoucements to a large list of friends, family, and colleagues regarding a small wedding. I would like to indicate somewhere on the announcement that I am taking my future husband's last name and would like to share our new home address (I don't believe the return address on the envelope is sufficient is it?). My quandry is, I am afraid for those who aren't quite close to us, if I don't incorporate my maiden name somehow, they won't know whom it is from! Do you have wording suggestions? Any help would be greatly appreciated.

Dear Enaols,

I suppose you could combine name, announcement, and the new address card. It makes sense these days. So, just use the traditional announcement form, stating that you and your fiance are now married. Then under that you could add a line stating that you will be taking your husband's last name and be known as Mrs. (Ms.) Jane Husband's Name. Under that state that your new address will be... . These do not have to be formal.
