Casual Wedding Invitations -- how to address outer and inner envelopes

the outer envelope should be Mr. and Mrs. John Smith and the inner envelope can be just: Patty and John Smith? or Patty and John? I need to provide this info to the calligrapher (not a professional, a friend). Our invitations are casual and our wedding is in the Florida Keys (we're in Palm Beach) so it's sort of a destination.

Yes, the inner envelope can be less formal, more familiar. Include the names of all invited guests on the inner envelope.
