Entree choice on response card

Do you have any advice on how to identify which person is choosing which entree on the response card?

Until very recently we didn't list any of this on response cards. But, it is necessary these days. Perhaps you could leave a line long enough for names next to the choices. You could even ask them to write their names next to their choices.
