How to assemble wedding invitations including enclosures

We chose a more formal invite with the papers in between, etc. What I am wondering is what specific order do they go in once inside the inner envelope? Is it appropriate to enclose a map/directions card along with menu choices? If so, where should they go amongst all the various cards? How are the invitations assembled?
