HI I am trying to figure out how to write a sentence in my reply cards such that the guests who are coming (can range from 1 adult on the invite to 4 adults) tell me which of 3 entrees they wish to have. This is the way it is written so far:

Indicate numbers for entrees below
Chicken ____ Salmon ____ Vegetarian _____

I am not sure of the grammar in the sentence, or if the instructions are clear eg are both words "numbers" and "entrees" plural as shown? Also is "for" to be used or "of".
