How do I address the inner and outer invitation envelopes to a married couple who are both doctors? I have one couple who are both medical doctors and another where the wife is a medical doctor and the husband has a PhD.

Dear Doctors in the House,
When there are two medical doctors, address both envelopes: The Doctors Smith.
Now not too long ago, I would have suggested to address the Ph.D as Mr. But, it is more and more acceptable to address Ph.Ds as Dr. This just seems fair. So, you would address them the same.
There is one other way to address the doctor and Ph.D. : Mary James, M.D (with his name under hers) John James, Ph.D on the outer envelope. The Doctors James and the inner.
