cake server as bridal shower gift

Who is responsible for buying the cake cutter/server, money/card bag and guest book? I am the maid of honor and I would like to buy them for the bride as her shower gift.

It is acceptable to purchase those items as gifts, just be aware that many people have similar inclinations. We see double sets of toasting glasses, for example, at many weddings. If there is a way for you to subtly ask the bride, or someone else close to her, what she had in mind for those things first, I would recommend it (especially as she has probably chosen colors and a theme, or at least a feel for the event already). Otherwise, feel free to gift, but be very understanding if she tells you at some point that she's received the same items from someone else and now needs to decide how to handle it.
It could be safer to get her a gift certificate for a very nice source of those items, and let her go to town - maybe she'll take you shopping with her!

Yes, it would be acceptable for you to purchase these items. But, you are not responsible for purchasing them. The couple is.
I know in some cultures and a few regions a money card bag is common, but in mainstream etiquette, it is considered inappropriate as it implies the couple expects their guests to give them money.
Also, I'm sorry but it really isn't appropriate, etiquette wise, to give gift cards for a shower. These gifts should be opened and a gift card isn't something that falls into that category.
