Defining the formality of a wedding

Can you clarify for me what aspects of a wedding put the wedding into the category of a formal or semi-formal wedding? Does the dress the bride is wearing set this tone or is it the bridesmaids dresses, what the groom is wearing? I know that the male members of the wedding party choose their attire based on the formality of the wedding but what defines the formality?
