My fiance's mother has been married twice and is now divorced. He was essentially raised by his mother's second husband and refers to him as his dad, although his mother isn't on speaking terms with him. We want to acknowledge his "ex-stepfather" in the Parents section of the wedding program. What wording can we use so as not to offend either his ex stepfather or his mother?

Unless he is seated as special or will be doing something during the wedding, he shouldn't be mentioned in the program. If he is seated in the third row as an ex-husband, he could be listed as father of the groom.
Your fiancé could also mention his stepfather during the reception.
